What is a Child Alert Request?

A Child Alert Request is a warning to the Department of Foreign Affairs and Trade that there may be circumstances which prevent the issue of an Australian passport or other travel document to a particular child.

Who can make a Child Alert Request?

You can make a Child Alert Request if you are the parent of, or person with a parental responsibility for, the child named on this form.

Validity

An Alert Request will remain valid for a maximum period of 12 months. Where a court order is provided, the Alert Request will remain in force until the child is 18 years of age, legally marries, or as directed by the court.

Requirements for printing Application Forms

It is now possible to download passport application forms in Portable Document Format (PDF). PDF files are readable using the Adobe Acrobat Reader which may be downloaded free using the following link: Adobe Acrobat Reader

The forms can be printed in colour or black and white on A4 plain white paper using a good quality laser or inkjet printer. Dot matrix printed forms or forms which are blurred or illegible in any way, cannot be accepted as they are unable to be scanned into the passport issuing system.

The following table sets out the printing requirements:

Paper Type	A4 plain white.	Thermal paper, dye-sublimation paper, special inkjet paper and other shiny papers.
Paper Weight	Between 80-120gsm.
Print Orientation	Portrait, single-sided only.	Double sided printing.
Print Quality	Normal to high resolution. Colour or black and white.	Quality of print should not be blurred or excessively light or dark.
Printer	Good quality laser or inkjet.	Dot matrix printed forms and blurred or illegible forms.
